There may be an old password cached in your device's settings/keychain. Try turning OFF the TV Provider setting on your device (iOS settings > General > TV Provider). Make sure you both uncheck TELUS AND click 'sign out'. When you try to log in to the TELUS TV+ app again, select 'no' when prompted with 'sign in with tv provider'. Then enter in your My TELUS username and password.
